I stared out the week a few weeks ago with an emergency stone extraction from the penis, right here in our Cysto suite. Uh yeah it's VERY uncommon to do this in any office without anesthetic (besides a urojet). Problem was the pt came in complaining of a lump in his Penis (urethra) and URO*doc decides to do a cysto to r/o urethral stricture as the pt was having a hard time voiding. Ok insert scope and not even 2 inches in is a very large stone. Obviously the scope wasnt passing and since we found the problem, Uro*dr asks the pt if he wants to go the hospital to have this removed...Pt declines and wants to know if we can just "take her on out here!" oh my, we really aren't equiped to actually do an in office stone extraction but what they hell, lets throw down all of the flex forceps we have and see what we can do!! Well let me tell you, it wasn't pretty but this pt was a trooper!! After about 20 minutes, two different flex forceps and a hemostat later, this stone was out and it was the size of a cherry pit if not larger! It was definitely the biggest stone i've ever seen and this poor pt was soooo happy he didn't have to go to the hospital!! Amazing, seriously this pt was amazing! Later, I stopped Uro*dr in the hall and remarked about how awesome that was and then happened to mention that we not try that again any time soon, he agreed!! lol

Wow! He's braver than I ever would be. Urodoc wouldn't even think he could do that with me. I mean not that I have a penis or anything... but not even in the urethra short version. No way!

Quick question: how much do your uro's operate? Sounds like they are mostly in office, but I assume they have to whack out the occasional, kidney, prostate, testicle in the OR. How many days a week do your guys operate i.e. not cysto-suite, full on OR? Do you assist when they are operating?

welcome! We have 3 docs in our very busy little office and they all do surgery at least twice a week. We see in our offices about 300 pts a week between all three and that does not include the hospital surgeries! We as MAs' assist in the office procedures I.E cystos, bl bx, prostate bx, vas and such but they have their "own" Surgical nurses and staff at the hospitals. As a medical assistant we are not licensed to be in the OR with them, but they definitely utilize us as much as possible in the office. Thanks for reading!

What does assisting consist of with the various procedures? How much do you actually get to do?
for an example, when we assist with stent removals, we had the dr the scope and once he has passed the sphincter and into the bladder, we visualize the stent and WE open and close the forceps for him, mostly it is handling instruments however, there are a few tests in the office that only the MAs do, Uro dynamics testing, cystometry voiding studdies, and bladder scanning just to name a few. We also help during biopsies to collect the samples to be sent for pathology... We actually do quite a bit :-)
